This is the website of the Commission for Continuing Education Provider Recognition and the CERP Program (collectively, “CCEPR”). This website is hosted by the American Dental Association ("ADA"). This Privacy Notice addresses the use and disclosure of information about you collected by virtue of your use of this website in connection with CCEPR. This Privacy Notice does not govern information collected by CCEPR or the ADA offline or on other ADA websites or webpages having separate privacy notices or not linking to this Privacy notice. Any data you submit on this website will be processed in accordance with this Privacy Notice and U.S. law, which may provide different protection from the laws of the country where you are located. CCEPR maintains an online directory of ADA CERP recognized continuing education providers containing some or all of the following information: the dates of their term of recognition, name of a contact person, a mailing address, telephone number, fax number and web address. Such providers may post their course offerings on a separate area of the CCEPR website. These web pages may include information you submit via our website, and are publicly accessible on this website.

With respect to your email address: if required by applicable law, you will be permitted to opt out of unwanted email. Unless otherwise permitted under this Privacy Notice or the ADA Privacy Notice, if applicable, we will not give or sell your email address to any unrelated entity outside CCEPR, the ADA, its service providers, subsidiaries and affiliated entities (such as state and local dental societies) without your consent. By entering your email address and other information in the publicly accessible directory of recognized ADA CERP recognized continuing education providers, you consent to the public disclosure of such information in the directory.

The ADA also occasionally hires other companies to provide services on its behalf, including, but not limited to, hosting of online resources, processing credit card transactions, packaging, order fulfillment, answering customer questions about products or services, consulting services, data modeling, printing, sending postal mail and processing membership and event registration. We will provide those companies with information they need to deliver the service.

The CCEPR and the ADA may use personal information in their databases to confirm your identity for recognition purposes or in relation to authorized or unauthorized use of ADA or CCEPR services, networks or intellectual property.

Your personal information may be provided to government agencies or others as necessary to comply with the law or in response to legal or administrative processes such as subpoenas. Your personal information may also be used to give notice to you in relation to any unauthorized use of CCEPR and/or ADA services, networks or intellectual property. While the ADA takes measures to protect your personal information, no security measure or network is entirely secure. Your password to access ADA.org should be complex and should never be shared with anyone, and should be changed from time to time. After you access ADA.org, you should log off and exit your browser so unauthorized persons cannot access the site.

On request we may provide site visitors with access to information that we may maintain about them, including financial information, transaction information (e.g., dates on which users made purchases, amounts and types of purchases), communications that the visitor has directed to our site (e.g., e-mails, user inquiries), and contact information (e.g., name, address, phone number).
